San Diego Health Connect is a not-for-profit Health Information Exchange (HIE) that serves the San Diego community to ensure that patients’ health information is securely available when and where it is needed. Electronic health information exchange (HIE) allows doctors, nurses, pharmacists, other health care providers and patients to appropriately access and securely share a patient’s vital medical information electronically—improving the speed, quality, safety and cost of patient care. SDHC offers query-based exchange which is used by providers to search and discover accessible clinical sources on a patient. This type of exchange is often used when delivering unplanned care.
The process is simple! Fill out our Participant Inquiry Form or email or call us for more information. Your organization will fill out a participation agreement, sign a Business Associate Agreement (BAA) and work with our team to determine the level of connectivity for your organization.

Your privacy is extremely important. Only authorized members of your healthcare team can view your health record. San Diego Health Connect offers a secure system for information exchange that complies with state and federal privacy laws. We undergo a rigorous certification process to ensure our systems can be trusted. Our system uses specific protocols and a secure network to keep your health record safe. With enhanced security protections above what is possible with paper records and more controlled access to patient health information, patient privacy is better protected than ever before.
No you do not. Your health data is included in the San Diego HIE automatically by your providers unless you opt-out. You can contact us to find out how to opt-out of the HIE.
