Clinical Query Portal
Subscribed providers can query patient data via the HIE portal through user login or SSO access. Users can search for a patient's medical history, labs, medications, procedures, discharge summaries and CIE information.
Social Drivers for Health
San Diego 211's Community Information Exchange (CIE) is integrated with the HIE Clinical Portal allowing providers to clinical and social data side-by-side.
EMS Connect
EMS Connect is San Diego Health Connects cloud-based platform that connects EMS agencies, hospitals, and community health organizations using the SAFR model to streamline data flow from the field to the hospital and back. Our EMSConnect Hub transmits pre hospital data directly to the ED, enabling hospital staff to prepare resources before the patient arrives. The platform closes long standing gaps by giving first responders critical patient information often missing during 9 1 1 calls, reducing delays caused by incomplete hospital intake data, and returning outcome and billing reconciliation information to EMS agencies after transport.
Event Notifications
Providers can subscribe to notifications for selected patients who experience a specific event (ED admit) or match a pre-identified criteria (3 ED admits in 30 days) or to track a cohort (diabetic population).
Public Health Reporting
SDHC facilitates Electronic Lab Reporting (ELR), Syndromic Surveillance, and vaccines to the public health department.
The SAFR Model
EMS Connect’s SAFR Model delivers seamless, end to end care coordination by connecting EMS teams and hospitals in real time. The platform searches the Master Patient Index at dispatch to surface critical clinical details before crews arrive. It alerts the receiving hospital during transport with live vitals, EKGs, and care notes that flow directly into the patients chart. After handoff, it files the complete EMS record automatically into the EMR, eliminating manual documentation. Finally, it reconciles demographics, payer data, and clinical outcomes back to the EMS agency, closing the loop and empowering data driven quality improvement.
