San Diego Health Connect delivers messaging and alerts functionality that helps providers coordinate patient care.

Alerts

San Diego Health Connect can alert you to transitions of care. When a patient you have a care relationship with visits the emergency room (ER) or is admitted to or discharged from a participating hospital, our solution notifies you by email or text message. These automatic notifications can be sent to designated providers, payers or caregivers.

This alerts functionality satisfies Meaningful Use attestation requirements for both hospitals and eligible providers. Once you’ve been notified of an event, you can access our Medical Records Exchange to view details about the care transition and see if your patient requires follow-up care.

Direct Secure Messaging

Direct Secure Messaging is a secure email for exchanging health information—a straightforward mechanism for transporting patients’ health information to other care providers over the Internet. More secure than fax and more cost-effective than telephone calls, secure messaging helps you coordinate care with providers outside of your organization.
